病案管理科在临床开展典型病例错误编码分析讨论会为进一步提升临床科室对病案首页中“主要诊断”及“主要手术操作”的正确选择与编码能力，6月1日下午，医务部副主任、病案管理科科长陆显义，医保办主任严艳及编码员走进ICU，针对ICU病案首页的正确填写开展讨论交流。
ICU的病人多为危重症患者，病情复杂，合并症多，主要诊断的选择与编码往往无从下手。此次围绕典型的错误病案，大家分别从编码工作、临床工作、医保管理、以及医政管理角度展开分析讨论。
编码员沈佳佳反馈了ICU医师常见的编码错误问题，ICU主任许玺认真分析了当前ICU医生在主要诊断方面选择的误区和原因，并表示今后将进一步规范病案首页的填写。针对特别复杂的病例，ICU将及时与医务部、病案管理科协调，不断提升病案首页填写质量。
经过讨论，大家达成共识：正确选择主要诊断的原则是统一的，即“对患者健康危害最大、消耗医疗资源最多、影响住院时间最长”这三个因素是主要诊断选择的依据。

医保办主任严艳表示，医务人员在选择疾病主要诊断时要实事求是，要以主要诊断选择为原则，避免根据权重大小选取主要诊断。
医务部副主任、病案管理科科长陆显义表示，病案首页的“主要诊断”及“主要手术操作”的正确选择与编码是决定DRGs入组的关键所在，临床科室务必正确选择。而其它诊断也应按照病情从重到轻的顺序排列。她要求临床科室按要求做好首页质控工作，反复学习，统一规范。
此次讨论会深度剖析了IUC住院病案中主要诊断选择及编码存在的问题，总结了主要诊断正确的选择原则及编码规则，进一步提高了首页填写质量，为我院稳步推进医保DRGs结算管理工作奠定坚实的基础。
随着DRGs工作的推进，首页的填写质量越来越重要。病案管理科的职能也在逐渐改变，正采取多种举措保障我院首页编码质量。当前，病案管理科编码员承担了病案首页编码质控工作，每月进行质控反馈。今年，病案管理科走进临床科室，派遣年轻编码员深入临床科室轮转学习，与临床医师面对面交流，协助临床正确编码。此外，病案管理科将不定期对临床进行编码知识培训，开展典型病例错误编码分析讨论会，切实保障首页填写的内涵质量。（图文/陆显义丁萍编辑/孙炜编审/杨居艳）
